What Is Crepitus?
While the word crepitus may sound unpleasant—often mistakenly associated with being decrepit—it simply describes any grinding, creaking, cracking, grating, crunching, or popping that may occur when moving a joint.
Crepitus can occur at any age, though it tends to become more common as we get older.
The sounds associated with crepitus can vary. Sometimes, they are muffled and only noticeable to the individual, while in other cases, they are loud enough for others to hear. This type of crepitus should not be confused with crackling in the lungs or other bodily sounds, which can also be referred to as crepitus in medical contexts.
Crepitus is a broad term with a few main causes:
- Gas bubble release: Air bubbles popping due to gas exchange within the joint capsule during movement or stretching.
- Tendon or ligament movement: Snapping or rubbing over bony structures, which may also create a grinding sensation.
- Joint-related crepitus: In some cases, caused by bony rubbing or compression within the joint itself.
Is Cracking and Crepitus Normal?
In most cases, crepitus or cracking is completely normal and harmless. Think of it like the noises household objects make during their normal function—a fridge hums, pipes rattle, and joints can crack. These sounds are often just part of our everyday biomechanics.
When Could Crepitus or Joint Noises Indicate a Problem?
There are a few situations where joint noises may signal an underlying issue:
- Pain associated with joint noises: If crepitus is accompanied by discomfort—such as pain in the hip or wrist—it could indicate a joint-related issue, tendonitis, or ligament strain.
- Locking or catching in the joint: If a joint locks up or catches during movement, particularly with an audible sound, this may suggest an internal joint problem.
In general, any joint noises associated with pain or restricted range of motion should be assessed by a healthcare professional.
The Role of Awareness in Crepitus Perception
Clinically, I often find that patients with pain in a specific area, become more hyperaware of any sounds coming from that region. Many report clicking, rubbing, or grinding noises they hadn’t noticed before. However, in many cases these sounds were likely always present, but the patient is only now tuning into them due to heightened awareness caused by pain and rehabilitation efforts.
I liken this to shopping for a particular model of car, once you start looking for a BMW, you suddenly see them everywhere, even though they’ve always been around.
What Are the Most Common Crackling and Popping Joints?
Some of the most frequently reported joints that produce crepitus include:
- Temporomandibular joint (TMJ)
- Knees
- Ankles
- Front of the hip – often due to the psoas or hip flexor tendon
- Mid to lower neck
- Wrist
- Shoulder – particularly around the scapula
